Geriatric care management includes emergency preparedness and other support
Geriatric care management services are provided by certified Aging Life Care Managers and their supporting teams. Some of the geriatric care managers work alone while others partner with home care organizations. Being a home care organization providing geriatric care management services, we are often asked to help keep the household in order, maintain and manage financial and legal paperwork including utility bills, and more complex tasks. The goal is to improve the clients’ quality of life and reduce family stress through assessment and monitoring, planning and problem-solving, education and advocacy as well as family caregiver coaching.
