Care Management
Starlight Caregivers offers certified, experienced and compassionate geriatric care management (GCM) services. Duanni Hurd, co-founder, president and CEO of Starlight Caregivers, is an Advanced Professional Member of the Aging Life Care Association®.
Professional geriatric care management is a holistic, client-centered approach to caring for older adults or disabled persons. The goal is to improve the client’s quality of life and reduce family stress through assessment and monitoring, planning and problem-solving, education and advocacy and family caregiver coaching.
Geriatric care managers assist in a variety of areas including but not limited to:
- Medical management: Attending doctor appointments; facilitating communication between doctor and client; monitoring the client’s adherence to medical orders and instructions.
- Communication: Keeping family members and professionals informed as to the client’s well-being and changing needs.
- Legal: Referring to or consulting with an elder law attorney; providing expert opinion for courts in determining level of care.
- Financial: Reviewing or overseeing bill paying or consulting with an accountant or the client’s power of attorney designee.
